Authorization to Release Financial Information

You must give written consent if you want the Office of Student Financial Assistance or the Office of Student Accounting Services to discuss any of your financial aid, scholarship or billing/account information with persons other than yourself by the following means:

• phone
• in person
• U.S. and electronic mail

Effective Fall 2007:
You, the student, must:

• complete the form in EKUDirect for immediate authorization:
  1. Login to EKUDirect
  2. Click on EKU eForms tab
  3. Click on and then complete "Authorization to Release Financial Information"

  or

• turn in a printed and completed  Authorization to Release Financial Information Form to the Office of Student Financial Assistance

This authorization form does not allow the university to release academic information.

You may revoke this authorization in writing to:
