Assets Form - Required for students/parents who had to complete a 1040 Federal Tax Return.

Computer Cost Allowance Form - Allows for a one-time maximum adjustment of up to $1,200 for the purchase of a computer and/or computer equipment while you are enrolled at EKU. Read Instructions.

Degree Status Form - inquires whether student has earned a bachelor's degree and which type of degree student is currently pursuing.

Dependent Care Expenses Form - Student may be eligible for dependent care expenses while enrolled if expenses are incurred for dependent care children or for an elderly/handicapped dependent. Click here for more information.

Drug Conviction Worksheet - required for students who report a drug related conviction on the FAFSA.

EKU Consortium Form - required for students enrolled concurrently at EKU and at another institution during the same term with EKU being the home school disbursing financial aid.

Graduate Enrollment Status Form - Graduate cost of attendance is based on enrollment of 6 credit hours per semester. If you enroll in more than 6 credit hours, you may request your cost of attendance to be adjusted accordingly.

Graduate PLUS Loan Application - required for students requesting a Grad PLUS loan.  Grad PLUS loans are for students enrolled at least half-time in a graduate or professional program who have exhausted their Federal Subsidized/Unsubsidized loan eligibility.

Household Size and Number in College Form - required if the number of family members in the household or number of family members in college reported on the FAFSA does not match the information reported on the Verification Worksheet.

PLUS Loan Denial Exemption - Dependent students may be eligible for additional unsubsidized loan amounts if the student can document exceptional circumstances which prevent a parent from borrowing a PLUS loan.

Post Bachelor Degree Form - required for students working on a second bachelor's degree, requires academic advisor confirmation of hours earned toward second degree.

Satisfactory Academic Progress Appeal Form - appeal form for reinstatement of financial assistance due to unsatisfactory academic performance.

Scholarship/Other Aid Resources Form - used to report up to six other financial aid resources which are not listed with the Student Financial Assistance Office.

Social Security Number Correction Form - Parent - requests correct Social Security number, name and date of birth for parent.

Untaxed Income Discrepancy Form - required if the amount of untaxed income reported on the FAFSA does not match the amount reported on the Verification Worksheet.
